  1. Nina Solyk was 87 years old when she died. She lived at Bayview Nursing Home, 1295

Frankston Dandenong Road in Carrum Downs. Her medical history included progressive dementia including aggression and threats, congestive heart failure, atrial fibrillation, oesophageal reflux and hypertension. :

  1. At 5.30pm on 7 March 2008, Mrs Solyk’s mental state deteriorated and-she was transferred by ambulance to Casey Hospital under section 9 of the Mental Health Act 1986. On 8 March, she was placed on an Involuntary Treatment Order and transferred to Amaroo Unit at Dandenong Hospital.

  1. On 11 March 2008, Mrs Solyk was diagnosed with a urinary tract infection and probably chest infection. She was treated with antibiotics but her condition did not improve.

  2. At 6.15am on 12 March 2008, nursing staff found Mrs Solyk unresponsive in bed, She was unable to be revived. :

  3. A recommendation that the medical investigation proceed by way of inspection was accepted by the Coroner. The forensic pathologist who inspected the body and the medical records formed the opinion that the cause of death was sepsis. Contributing factors were dementia, congestive heart failure and cerebrovascular disease.

  4. Accordingly, I find that Nina Solyk died of sepsis.
