in the following circumstances:

  1. Jamie Alan White was 44 years old when he died. He lived at in a residential care unit managed by ParaQuad Australia at 37 Murray Street in Coburg. Mr White’s medical history included neurofibratomatosis and C5/6 parapalegia following a C2/C6 laminectomy in 1997 as well as depression, deep vein thombosis, pneumionia, sleep apnoea, chronic severe referred pain and respiratory issues. His general practitioner was Dr Joseph Toscano. Mr White was prescribed oxycontin, mirtazepine and warfarin. On 12 June 2008, Mr White was also treated for a urinary tract infection at the Austin Hospital,
