AND having held an inquest in relation to this death on 3 February 2010 at Melbourne
find that the identity of the deceased was Gladys Emma Vincent and the death occurred on 13 March 2009 at GO Community Residential Care Services 39 Cleveland Street, St Albans.

Ms Gladys Vincent was born on 7 September 1948, and she was 60 years of age at the time of her death.: Ms Vincent was a recipient of disability residential services provided by the Department of Human Services, pursuant to the Disability Act 2006, She was intellectually disabled and a tesident of Go Community Residential Care at 39 Cleveland Street, St.Albans.
She had previously resided at Kew Residential Services. Ms Vincent was a person in care as defined by s3(d) of the Coroners Act 2008 ("the Act") and accordingly her death was reportable pursuant to s5(c) of that Act. An inquest into her death is mandatory pursuant to s52(2)(b) of the Act. This inquest has procceded by summary of the circumstances.

Ms Vincent’s death was due to natural causes. Ms Vincent was a long time resident of disability services. Medical records revealed an extensive history of medical treatment. She had been diagnosed with renal carcinoma two years previously. She was under the care of Western Hospital Sunhine and Western Palliative Care, and her General Practitioner, Dr Huynh of St.Albans. At the time of her death she was being treated palliatively, including measures to alleviate pain. Ms Vincent’s condition deteriorated over the three days prior to her death and she died on 13 March 2009,
An autopsy conducted by Dr Shelley Robertson, Senior Forensic Pathologist with the Victorian Institute of Forensic Medicine, reported that the cause of death was renal carcinoma and that there contributing factors were Anaemia, Ischaemic Heart disease, Smith- Magenis Syndrome and Intellectual Disability. The pathologist reported that there was no evidence that the death was due to anything other than natural causes.
Having considered the available evidence, I am satisfied that Ms Gladys Vincent died on 13 March 2009, from natural causes. The cause of death being Renal Carcinoma, Other conditions including Anaemia, Ischaemic. Heait Disease, Smith-Magenis Syndrome and Intellectual disability were contributing factors.
