Brian Anderson was 50 years old when he died. He lived in a DHS Support centre managed by Australia Independence Community Solutions at 77 Mount Dandenong Road in Ringwood East.

Mr Anderson was a quadriplegic as the result of a driving accident in 1985. He had also recently " suffered from a chest infection. His general practitioner was Dr I Sagi. On 4 August 2009, Dr Sagi prescribed antibiotics and a Ventolin puffer.

At about 9.00am on 12 August 2009, a personal care worker, Jana Majdalani, woke Mr Anderson and placed him on the commode. At about 9.05am, Mr Anderson said his chest felt awful as if his chest infection had returned. At 9.10am, Mr Anderson called out and staff noted his lips were bluc and he had chest pains.

Mr Andrews than collapsed. Ambulance officers did not attempt resuscitation at Mr Anderson’s prior request.

The forensic pathologist who performed the autopsy formed the opinion that the cause of death was coronary artery disease in the setting of double vessel coronary atherosclerotic disease and cardiomegaly.

Accordingly, I find that Brian Anderson died from coronary artery disease in the setting of double vessel coronary atherosclerotic disease and cardiomegaly.
