  1. Wladislaw Kowalczyk was aged 62 years at the time of his death. He lived in supported residential care at 5 Ramleh Road, Reservoir.

"Mr Kowalczyk was a mentally handicapped male and lived in a supervised Department of Human Services premises at 5 Ramleh Road, Reservoir. On the 23rd April, 2010 he presented at the Austin Hospital Emergency Department suffering breathlessness. He was examined and diagnosed with chronic heart disease and multi-organ failure, At 1600 on the 10th May, 2010, he was transferred to the palliative care ward at the Austin Repatriation Hospital. On the 11th May, 2010, he complained of chest pain and

breathlessness. He was administered with morphine and midazolam and passed away at 3.15pm on the same date (11/5/10)."!
