AI-generated summary

Henry Portelli, a 76-year-old prisoner with significant cardiovascular disease including cardiomegaly, myocardial fibrosis, and ischaemic coronary artery disease, died of cardiac arrest in custody. The autopsy showed chronic structural heart disease sufficient to cause spontaneous lethal arrhythmia. He had comorbidities of diabetes and renal impairment. The coroner found no acute ischaemic changes on myocardial examination. Key clinical lesson: prisoners with known cardiac risk factors require appropriate cardiovascular assessment, monitoring, and management to mitigate acute decompensation risk. Access to timely medical care and cardiac intervention protocols within custodial settings is essential.
