I, PETER WHITE, Coroner having investigated the death of LEON THOMPSON

AND having held an inquest in relation to this death on 21st March, 2012 at Melbourne

find that the identity of the deceased was LEON JAMES THOMPSON born on 16th August, 1946 and the death occurred between 17th and 18 January 2011

from: la. ACUTE ANTERIOR MYOCARDIAL INFARCTION 1b. CORONARY ARTERY OCCLUSION

in the following circumstances:

  1. On Monday the 17th of January, 2011 Mr Leon Thompson went to bed in his room at the Bailey House Residential Care Facility located at 39 Lubrano Street, Brighton East.

  2. At 1.00am on Tuesday the 18th of January, 2011 Lorraine Hyland, the supervisor on duty at Bailey House, entered Mr Thompson’s room to check on him. Ms Hyland noted that Mr Thompson appeared to be sleeping normally, Ms Hyland ensured a bell used by Mr Thompson to attract staff attention was beside his bed should he need it.

  3. At approximately 5.30am on Tuesday the 18th of January, 2011 another resident of Bailey House, Mr Anthony Vincent, went to Mr Thompson’s room to wake him for breakfast. Mr Vincent located Mr Thompson sitting slumped in an arm chair outside Mr Thompson’s bedroom. Mr Vincent attempted to wake Mr Thompson and when he was unable to do-so he notified Ms Hyland.

  4. Ms Hyland examined Mr Thompson and being unable to wake him called ‘000’, Ms Hyland was directed to commence CPR on Mr Thompson which she did until the arrival of members of the Metropolitan Fire Brigade.

  5. Metropolitan Fire Brigade members attended Bailey House as did members of the Metropolitan Ambulance Service, Staff from both agencies confirmed nil signs of life in Mr Thompson and Police were then notified. ,

  6. Police attended Bailey House and spoke to both Mr Vincent and Ms Hyland. No suspicious circumstances were identified. Ms Hyland provided details of Mr Thompson’s medical history including that he had suffered a heart attack on the 31st of December, 2010, leading to admission to the Afred Hospital for 6 days. Mr Thompson also suffered from High Blood Pressure and High Cholesterol for which medication had been prescribed. Mr Thompson suffered from diabetes which was being managed through a specialised diet. Warfarin was also prescribed to Mr Thompson after suffering a heart attack in December 2010.

  1. Following examination, the cause of death of Mr Thompson was found to be Acute Anterior Myocardial Infarction and Coronial Artery Occlusion. I adopt this finding.
