I, AUDREY JAMIESON, Coroner having investigated the death of WAYNE EADIE

AND having held an inquest in relation to this death on 9 May 2013 at MELBOURNE

find that the identity of the deceased was WAYNE PAUL EADII born on 17 March 1964

and the death occurred on 13 February 2012

at Northern Hospital, 185 Cooper Street, Epping 3076

to

1 (a) PNEUMONIA

in the following circumstances:

I. Mr Wayne Eadie was 47 years of age at the time of his death. He had lived at 5 Henderson Court, Plenty Residential Services.in Bundoora since August 1997, having previously lived at

Janefield Training Centre between 1964 and 1997, Mr Eadie had an intellectual disability

from birth and never learned to speak, He had been « person placed in custody and care as it is defined by section 3 of the Coroners Act 2008,-all of his life. Mr Eadie’s mother also has

an intellectual disability and had been unable to care for her son.

2, Mr Eadie also had a medical history of bowel cancer, diagnosed in 2008. He underwent surgery to remove a large tumour and a portion of his lower bowel and, as a consequence, was fitted with a colostomy bag. In 2009 he was diagnoséd with renal failure and was fitted with a suprapubic catheter and urine bag. He had been diagnosed with epilepsy as a child and was reated on anticonvulsant medication all of his life. His seizures were reported to be

infrequent. His medications included Largactil 75mg three times per day, Tegretol 200mg

wice per day, Ostelin daily and Ranitidine 75mg twice daily.

Surrounding Circumstances

  1. On3 February 2012, Mr Eadie appeared to be unwell and was taken to see his general medical practitioner, Dr Lyndsey Kabat at Epping Healthcare Centre. Dr Kabat recommended that Mr Eadie be transferred to the Northern Hospital, where he was subsequently admitted, and

diagnosed with bilateral severe community acquired pneumonia. He was treated with broad

spectrum antibiotics. His condition however continued to deteriorate and he developed acute

on chronic renal failure. Mr Eadie died on 13 February 2012.

Investigation

  1. The identity of Wayne Paul Eadic was without dispute and required no further investigation.

Medical Investigation

  1. Dr Paul Bedford, Forensic Pathologist at the Victorian Institute of Forensic Medicine, performed an external examination on the body of Mr Eadie and reviewed a post mortem CT scan. On the basis of information available to Dr Bedford, he reported that on a background of history intellectual disability, Mr Eadie had developed community acquired pneumonia

which led to/his death.

Police Investigation

  1. The police investigation and preparation of the coronial Brief of Evidence was prepared by

Senior Constable J. Tinker of Epping Police Station.

Inquest

7, An inquest was held in accordance with section 52(2) Coroners Act 2008 as. immediately

before his death, Mr Eadie was a person placed in custody or care as it is defined in The Act.

  1. No issues were identified in the course of the investigation and a summary of the evidence was provided to the Court by Sergeant Sharon Wade from the Police Coronial Support Unit,

assisting the Coroner.

Finding I FIND that Wayne Paul Eadie died from natural causes being pneumonia.

AND I further FIND that there is no relationship between the fact that Mr Eadie was a person

placed in custody or care and the cause of his death.
