having investigated the death of MR JOHN MACKENZIE
AND having held an inquest in relation to this death on 8 AUGUST 2013 at MELBOURNE
find that the identity of the deceased was JOHN MACKENZIE
born on 6 MARCH 1962
and the death occurred on 2 OCTOBER 2012
at THE BASIN, VICTORIA
from:
LA, COMPLICATIONS OF END STAGE RENAL FAILURE (PALLIATED) IN A MAN WITH DOWN’S SYNDROME AND POST TRAUMATIC STRESS DISORDER
in the following circumstances:
A summary inquest was held in relation to the circumstances of the death of Mr John Mackenvie in circumstances where Mr Mackenzie was in care. The inquest is a mandatory
inquest.
Mr Mackenzie was 50 ycars of age at the time of this death. He resided in a care facility and had been in the care of the Department of Human Services (‘DHS’) since the time of his
birth.
At the time of his death, Mr Mackenzie resided at 12 Arcadia Avenue, The Basin, a facility
operated by DHS. He had been residing at this location for approximately 21 ycars.
Mr Mackenzie had a medical history of Down’s Syndrome, severe intellectual disability, celiac disease and post traumatic stress disorder. He had a recent medical history and had been admitted to the Angliss Hospital with a diagnosis of pneumonia and chronic renal failure. He was discharged from hospital for palliation on 19 September 2012 in renal
failure.
Mr Mackenzie was managed palliatively in the facility and medicated in that context. He was located on Tuesday 2 October at approximately 5.00 a.m. by his carer, unresponsive.
An ambulance was called and he was unable to be resuscitated.

- Dr Jacqueline Lee, forensic pathologist with the Victorian Institute of Forensic Medicine, has reported that the cause of death was Complications to End-Stage Renal Failure
(Palliated) in a man with Down’s Syndrome and Post Traumatic Stress Disorder.
- In those circumstances, I am satisfied that the cause of death was natural. There are no suspicious circumstances. | am satisfied that the medical treatment and management and
care provided to Mr Mackenzie was reasonable and appropriate.
- T direct that a copy of this finding be provided to the Interested Parties and to the
Investigating Police Officer.
