I, JOHN OLLE, Coroner having investigated the death of ADANG AKOT

AND having held an inquest in relation to this death on 12 June 2013 at MELBOURNE

find that the identity of the deceased was ADANG AKOT

born on 29 Janmary 2012

and the death occurred on 3 October 2012

at The Royal Children’s Hospital, 50 Flemington Road, Parkville 3052

from;

l(a) INTERCURRENT PNEUMONIA/RESPIRATORY INFECTION

2 CHRONIC LUNG DISEASE AND SUBGLOTTIC STENOSIS

in the following circumstances:

1, Adang was born at the Royal Women’s Hospital on 29 January 2012 and was subsequently transferred to the Royal Children’s Hospital.

2, The circumstances of her death are the subject of inquest by virtue of her ‘in care’ status pursuant to the Coroners Act 2008. Adang was subject to an interim accommodation order, Department of Human Services.

3, I have received reports of the Department of Human Services and the Royal Children’s Hospital.

4, The above reports are contained in the inquest brief,!

  1. 1 am satisfied that the Royal Children’s Hospital report has accurately set out the clinical course from the date of Adang’s birth until her death on 3 October 2012. I am satisfied there are no protective concerns or medical management issues identified.
